Chronic Disease Management
Chronic diseases are complex and can cause great distress to the individual and their families. If managed appropriately and well however, they can be controlled adequately, keeping complications at bay and improving quality of life
Common chronic diseases include diabetes, osteoarthritis, emphysema, chronic bronchitis, congestive heart disease, hypertension, cardiovascular disease, COPD, asthma and depression. A chronic disease management plan ensures that the patient’s regular GP is taking a holistic and proactive approach to their care and ensuring a team of allied health experts are consulted to improve quality of life. This approach may allow access to subsidised allied health visits for the patient. Ask our team of GP's about how better to manage your chronic conditions.
